If the veterinarian managing your dog’s care decides that your dog would benefit from canine rehabilitation, a referral is needed. Our therapist will keep your vet updated on your pet’s progress and any medical concerns that arise.

The referral form below can be filled out and emailed to info@southsoundk9rehab.com. If emailing the client’s file, the referral form would only need client information, diagnosis, and provider signature. Use of our referral form is not required.
